I, the parent or legal guardian of the participant named in this form, give permission for my child to participate in the Youth Football Camp listed in this registration. By signing this form, I acknowledge that football and related camp activities involve inherent risks, including but not limited to falls, collisions, contact with other participants, strenuous physical activity, and other risks that may result in injury, illness, disability, or death. 

I understand these risks and voluntarily allow my child to participate. I certify that my child is physically able to participate in camp activities, except as disclosed by me in writing on this form.

In consideration for my child being allowed to participate, I release and hold harmless the camp organizers, host school or facility, coaches, volunteers, sponsors, and affiliated staff from claims arising out of ordinary negligence related to my child’s participation, to the fullest extent permitted by law. This type of release and hold harmless language is commonly used in youth camp waivers, though enforceability can vary by state and may be limited for claims involving minors.

I authorize camp staff, athletic trainers, emergency personnel, and licensed medical providers to obtain or provide medical care for my child if I cannot be reached in an emergency. I understand that I am financially responsible for medical treatment provided to my child.

 

I certify that I am the participant’s parent or legal guardian and that I am at least 18 years old. I have read this document, understand it, and agree to it voluntarily.
